Residents/Businesses/Government Offices within the Dinalupihan Water District’s area of jurisdiction

WHAT ARE THE REQUIREMENTS?

A. VALID IDENTIFICATION

For Applicant

  • Original and photocopy of 1 valid ID
  • DWD Statement of Account or Official Receipt of nearest neighbor
  • Tax Declaration (photocopy) or Barangay Residency

For Representative

  • Original and photocopy of 1 valid ID
  • Original and photocopy of 1 valid ID of representative
  • DWD Statement of Account or Official Receipt of nearest neighbor
  • Authorization letter from the applicant

B. PROOF OF OWNERSHIP

  1. Photocopy of lot title; or
  2. Tax Declaration; or
  3. Barangay Lot Certificate
